To All Referrers

When referring a patient please indicate how you can receive the report on the referral. Please give us as much information as possible. Options are

1. as normal
2. phone call, please indicate a cell phone number
3. snail mail ie letter but this will be delayed
4. urgent written report given to the patient
5. fax
6. email
